I am 25 and have PCOS. I did my second round of clomid this month at 100mg. I had a day 21 progesterone test (actually on day 23 because day 21 was a Saturday) and was told I did not ovulate. I also did not ovulate last month on 50mg. However, my nipples became sore on day 22 (day before the test) and are still sore now on day 29. This has never happened to me before, but I have read that this type of soreness can be caused by a progesterone rise after ovulation. So, I was wondering if I could have possibly ovulated a day or two before the blood test and, therefore, still had a low level or if there is another reason for the soreness I am experiencing. I rarely have periods on my own (about 3 per year) and have had to use prometrium to start a period for both of my clomid cycles. So, if I have a period on my own, this time, would this mean that I did ovulate? I am currently seeing my Ob/gyn and plan to do one more cycle of clomid with him and then see and RE if I don't get pregnant.

First, you have to make sure you definitely have PCOS - What type of workup/testing did you have done to assure that's your diagnosis? What were your TSH and prolactin levels? Fasting glucose/insulin levels? Did your doctor discuss the potential medical and reproductive ramifications of PCOS with you?
You're right - If you ovulated right before the blood test, your progesterone level may have been on its way up (just starting to rise) when you had the blood test done, and therefore may have still been "low". It's best measured about a week after ovulation.
How were you monitored on your Clomid cycle? What was the thickness of your uterine lining after taking the Clomid? Did the ultrasound show a dominant follicle? Did your estradiol level rise in your blood during your Clomid monitoring, or did the level stay low?

The only monitoring I have is a day 21 progesterone test and a transvaginal ultrasound and exam by the doctor on day 35. My doctor did test my prolactin and TSH levels before my first round and said they were normal. I did not have my insulin levels tested. He told me after my last clomid round (on day 35) that the pictures from my ultrasound showed what he would call the "classic" pictures of PCOS ovaries. I was also told that day that my lining was "thin" "but nothing specific about how thin and that there were "tons" of "small" follicles on my ovaries. I have asked about having my insulin levels checked and about metformin and was told that we would look at it if clomid didn't work alone.
