Dear Dr Scott,
Am 35 yrs. had an ectopic - tube rupture 2003. since then never been able to conceive. tried naturally for around 4 years, but in vain.
Start fertility treatment 2006.
Husband good responding doner, without history. everything fine on his side. blood test, sperm test etcc. everything OK.
Me too no leak in right tube. no diseases. blood test no problem.
I started with serophene for 2 cycle last year which did help at all.
then was on clomid had 3 cycles. on third cycle decided to have an IUI. since i had around 8 follicles 16-20mm on day 12 . After an hCG 10000 USP . I had an IUI.
when doctor showed be those fllicle , i was much confident that i will succeed. BUT in vain i didn't.

On my next cycles doctor discovered 5 cysts. I was put on rest... until they dissapear......

I had a LAP last month september. everything was ok.
So my Dr start with a new cycle FEMARA. I responding only 3 follicles of 21mm on day 11.
IUI was cancelled because not enough eggs.
NOw am waiting for my next cycle.

DR SCOtt.. I would like you to tell me please if it is possible for me to have a combination of hCG + hMG + E2.
I am really desperate, all my strenght are diminishing days after days... I feel exhausted..... I just want to hold my baby. My husband and I are ready to undergo any treatment, even if causes of multiple pregnancy .

Can anyone who takes femara and would she be able to have this combination hCG + hMG + E2.?
because I read many research which says that many woman with this combination succeeded in getting pregnant.
All relatives to whom I talk about my infertility, they get pregnant and hold their beautiful babies. am happy for them, god has blessed them.
Its only me who is left behind.

I have read that:
In most cases, doctors who do two IUI's do the first about 24 hours after the HCG shot and the second about 48 hours after the shot. Some studies have shown that doing one IUI about 36 hours after the HCG is equally effective. However, some recent research suggests that higher pregnancy rates may be achieved by doing two IUI's, one at 12 hours past the hCG shot and one at 34 hours.

My question is : would the two hCG same usp units. can someone get 2 x 10 000 usp units.???
Or would she get less usp units and how much should she get ???

Hi,

First, while your relatives have babies, let me say that you're NOT the only one who is "left behind" without a baby -- Up to one in five married couples in the U.S. have trouble having a baby. Have you considered joining an infertility support group?

To answer your question - Some doctors use hMG and hCG alone without a drug like Femara, while others combine Femara with hMG and hCG. Are you seeing a Board Certified Reproductive Endocrinologist?

I can't diagnose your condition, but will generalize in saying that many women with tubal pregnancies have abnormal tubes and sometimes are best served by undergoing IVF -- Has your RE discussed this with you?

Doctors don't give two hCG injections of 10,000 units to trigger ovulation - They only give one.

In normal menstrual cycles, gonadotrophin releasing hormone (GnRH) secretion is pulsatile, with intervals of 60-120 minutes in the follicular phase. Treatment with pulsatile GnRH infusion by the intravenous or subcutaneous route using a portable pump has been used successfully in patients with hypogonadotrophic hypogonadism. Assuming that the results would be similar in women with polycystic ovary syndrome (PCOS), pulsatile GnRH has been used to induce ovulation in these women. Although ovulation and pregnancy have been achieved, the effectiveness of pulsatile GnRH in women with PCOS has not been clearly demonstrated.
