  • Should I have a lap before trying injects?

    I have done clomid, femara, and now both drugs together. I'm in the 2WW so don't know for sure yet if this one worked, but my lining was a little thin, so I'm not too hopeful. My RE has suggested lap in January to explore and see if there's anything wrong in the pelvis. I guess he's ruling out endo, which I have no symptoms of, but I know that doesn't matter. Is it worth trying a cycle of injects before the lap, or should I just have the surgery. I'd really love to avoid the misery of surgery if possible. My DH has been tested and has no problems. I have PCOS, hypothyroidism, and insulin resistance. I'm on synthroid .025/day and metformin ER 750mg 2x/day. I had an HSG about 6 months ago which was clear. I'm 36 and have been TTC our third child for 17+ months. Thanks!!

    Hi,
    While it's illegal for me to tell you what to do next, I can generalize and say that many RE's are getting away from laparoscopy, especially when patients don't have pelvic symptoms or a family history of endo. Also, I've not heard of anyone combining Femara with Clomid before.
    There may be other factors about you that are working against your fertility that haven't been addressed, and those need to be looked at to find out why you haven't conceived again. As an example, prolactin levels may be elevated. DH may need to have a "strict" or Kruger morphologic classification of his sperm. Sperm DNA can be checked. You might be obese, and obesity most definitely can play a role in getting and staying pregnant. Have you considered a second opinion from a different, board certified RE?

      Thank you Doctor! I am a little overweight, but not obese. I'm pretty sure we've checked all bloodwork and that's been fine as far as I know. I recently had my TSH redone and it was fine as well. I responded well to the clomid+femara combo with 4 follicles last month and 3 this month. Just no conception, depsite IUI's that were well timed with good numbers on my husband's side. Well, I don't know about this month yet. I do have a 2nd opinion scheduled for January, so I might just hold off until then before deciding what to do next. Also, is it standard protocol to day 3 bloodwork, and also day 14 bloodwork to check e2 levels? My RE doesn't do that at all. I wonder if we are missing something there.

        Yes, it's standard protocol to check day 3 blood testing, especially at your age. I don't see the utility of a day 14 E2 level except for monitoring to see if/when someone may be close to ovulating.

        Good luck with your second opinion -- Make sure it's a Board Certified RE!

          Okay, those were only done one time and not since then. Does the FACOG after his name mean he's board certified? Jeffrey A. Keenan M.D., F.A.C.O.G.

            The FACOG means he is board certified in General Obstetrics and Gynecology. However, Dr. Keenan, from Tennessee, is also board certified in REI.

              Ah, okay. So he is certified. Thank you for that info!! I am seeing Gayla Harris in January for a 2nd opinion.
