I'm 31 years old and diagnosed with low egg reserve. We've been trying to concieve for over 4 years using Clomid regimes that eventually led to one IVF cycle that produced an empty follicle. As we're starting our second IVF cycle, I'm wondering what are the odds that I will be able to concieve? Between everything I've read and both RE's opinions I've received, there's been nothing but doom and gloom. Is there any possibility of a pregnancy using my eggs? Is this cycle even worth it?

Hi,
In my expert opinion, it's folly to give Clomid to someone with diminished ovarian reserve, especially for IVF. What did your RE say about the option of DHEA to improve ovarian reserve? Acupuncture? What did your fragile-X blood test show? AMH? Antral follicle count?

The Clomid was given by my OB/GYN. After 2 years of that, I requested to attend an RE who gave me the AMH test, and forgive me, I can't remember the exact number, but it showed that I most likely had low ovarian reserve and I'd be lucky to produce 2 or 3 eggs for retrieval. For the first IVF cycle, I was not on DHEA (I am on 50mg/d for this current 2nd cycle) and my estrodial levels were low in responding to the injections; 31.5, 79.3, 128.6, 161.6, 218.1 two days before harvest. The ultrasounds showed one follicle with a mean of 16.8 two days before harvest and other smaller developing follicles on the left ovary.
Because they weren't able to harvest an egg the first round of IVF, my RE suggested obtaining a donor egg. I gained a second opinion from another certified RE and he said pretty much the same thing.
I don't know that I had a fragile-X blood test. What is that?

Fragile-X is a blood test we do on our patients. If it's abnormal, it can explain DOR.
When we do IVF at my center, we want to "do it once, do it right, and make it work". With that in mind, we pull out all stops right from the start to optimize the odds of success. One of those many things we do is acupuncture, starting BEFORE the IVF cycle even begins. The published DHEA protocol is higher than 50 mg/day....
