    Dr Roseff,

    I am trying to determine where to go from here. I have had a total of three miscarriages. The first was in 9/2007 and my OB advised that it was a blighted ovum. I was 9 weeks at the time. I got pregnant again early 2008 with my daughter. I had a normal pregnancy until 9/2008 and was put on bed rest at 32 weeks due to excessive contractions. I was also put a medication to prevent contractions that were occurring as close as every 2 to 5 minutes. At 36 weeks I went into labor and delivered a healthy baby girl.

    In 2009 my husband and I decided to try for another and became pregnant. In 8/2009 I had another miscarriage at 9 weeks. We did see the yolk sac, but never heard a heartbeat. After the second miscarriage my OB ordered some lab work and tested my thyroid and a Lupus Anticoagulant Screen. I was then referred to a specialist to review the results. The Lupus Anticoagulant Screen did come back positive, but I was advised that the number was so close it could have just fallen in the margin of error on the lab. The specialist would not see me as a patient. She said that she would do an evaluation once the pregnancy was established, but I would see my regular OB for appointments.

    November 2009 I had surgery for ovarian cysts (one on each ovary). My left ovary was removed completely with the dermoid cyst. My Dr did not try to remove the cyst on the right side as she said that it was too small.

    We decided that we would try again this year. In February we became pregnant. At the first ultrasound we heard a heartbeat and I was advised that the chances of a miscarriage dropped drastically after the heartbeat is heard. At my next appointment my Dr decided to do an unscheduled ultrasound to easy my mind that everything was okay. (I have had issues in the past with Generalized Anxiety and Panic Attacks.) When she did the ultrasound she could not find a heartbeat. I was between 9 and 10 weeks when this miscarriage happened. After this miscarriage my OB advised that I could go for fertility testing if I wanted too.

    Questions:

    1) Should there have been more testing for the Lupus Anticoagulant Screen?
    2) Should I be seeing a specialist rather than my OB?
    3) What is the likelihood that additional testing will be able to provide answers?
    4) Is there hope that I can still have a healthy pregnancy? I would like to try again, but I have a tremendous amount of fear of another miscarriage.

    So sorry to hear about your losses.... To answer your questions:

    1) Yes (as well as other reasons for loss)!
    2) Yes!
    3) High.
    4) Yes!

    Make sure you see a board certified RE who is an expert in RPL. If you can't find one near you, you're welcome to come here (as others have from other states and countries)....
