  • High FSH with other problems

    I'm 35 and my husband and I have been unsuccessfully trying to conceive for 18 months. We recently starting seeing an RE and have gone through the initial testing (sperm analysis good, HSG shows open tubes, cycle monitored through ultrasound and blood tests - ovulation around Day 17 - 2.1 cm follicle day of LH surge (Day 16) and ovulation confirmed via ultrasound 2 days after Lh Surge - showed follicle released, other blood tests normal).

    The problems found are:

    1. (via sonohysterogram) that I have 2 uterine polyps - 1 is 2.1 cm (0.8 inches) and another much smaller that will be removed via operative hysteroscopy.

    2. I have very low progesterone 7 days post ovulation (5.68 ng/ML or 18.06 nmol/L) and a shortish luteal phase (10 - 11 days) with spotting for several days

    3. Very concerning is FSH of 12.9 on Day 3 as well as an Antral Follicle count of 6. I will soon have an AMH and fragile x blood test.

    Also through these procedures I found I have a very, very tight cervix that made inserting the catheter extremely difficult (near impossible) and painful, although the RE said this makes it difficult for the doctor, however would not cause my infertility problems.

    Based on FSH and AFC, I understand that it may be more difficult for me to become pregnant after my problems with the polyps and low progesterone are treated.

    Is there any point in trying IUI for a couple cycles or would it be best to jump right into IVF?

    My doctor also wants me to have a Lupron Depot shot 1 month before the hysteroscopy and I think the reason is to thin my lining (I have a lot of spotting from the polyps and/or low progesterone - lining was 1.3 cm mid-cycle). Reading up on this, I really don't want this shot and will discuss this at my next appointment. Is there another drug I could take if the doctor insists on something (I suggested the birth control pill, but was told that would make the problem worse).

    Hi,

    Sorry to hear you're having problems. There's a strong possibility of endometriosis here, so discuss this possibility (and it's implications) with your RE. I'm surprised, frankly, your RE didn't make the connection between elevated FSH, uterine polyps, a tight cervix, and the possible correlation with endometriosis.

    I don't suggest pre-treating hysteroscopy patients with Lupron Depot. It's almost unheard of these days... Are you SURE you're seeing a real, board certified RE? Most docs simply have their hysteroscopy patients come in after menses has ceased and before ovulation has taken place. Alternatively, other docs use meds like progestins to thin out a uterine lining (if necessary). Has your doc discussed alternatives such as these with you?

    An ultrasound after presumed ovulation would not be necessary if we're checking a blood progesterone level. If the blood test shows ovulation took place, why do an ultrasound?

      Thank you very much for your response.

      Interesting that you would mention endo. I didn't know that any of those things could be associated with endo, but figured since I was going to be under anaesthesia already for the hysteroscopy, having a lap at the same time to rule out any possible endo might be worthwhile. The RE said no, because they don't like to do unnecessary laps in patients without symptoms (I do have mild cramps that start the week before my period and them more severe cramps that usually end after the first day of menses). Now I'm wondering if I might actually have endo.

      As for being board certified, I'm not in the States, so I don't think they have the same certifications here (unfortunately).

      I had already decided I did not want the Lupron Depot and your answer solidifies that choice even more. No other options were discussed but I will discuss this at my next appointment. I did not know what Lupron Depot was (other than it was an expensive shot) until after I left the appointment and researched it.

      Why bloods and ultrasound were done to confirm ovulation - I think they did this for the investigative cycle and am not sure if it would be done every month. However, our public health insurance covers all blood tests and ultrasounds as long as it is not an IVF cycle so it seems they do a lot of them. They require blood work at each and every cycle monitoring appointment (and other clinics here are the same). I had my blood drawn 10 times that month (initial infectious screening, Day 3, Days 10 - 16 due to the fact I ovulate late and then 7 days post ovulation). I would not be able to do cycle monitoring every month if it required that many blood draws as I only have one decent vein and was in terrible pain and badly bruised by day 14 and it only got worse.

      Thank you very much for your time. I am considering getting a second opinion, but it is difficult. After the required referral from my family physician the wait to see another doctor is usually 3 - 6 months and at this point, with my results, I realize waiting that long may be too late for us.
