    hi dr roseff,
    i am 45 and would like to know if one does iui with ds,in a natural cycle, would it help to also get vaginal insemination done with ds prior to iui just to ensure i dont miss the egg incase i ovulate earlier?

    also, if sperm can survive in vaginal mucus for up to 5days, why cant they survive in the uterus for even 6hrs? isnt there mucus inside the uterus as well.

    Hi,

    No, it's not recommended to have vaginal insemination in addition to IUI. IF the IUI is timed correctly (to the time of ovulation), then putting sperm in the uterus has actually been shown to have the sperm go right into the tubes as well. That gives us the highest odds of success and makes vaginal insemination unnecessary.

    No, there is no mucus inside the uterus.

      thank you for yr prompt reply.
      pls tell me, does the hcg shot gurantee ovulation? also, doesovulation occur after 36hrs mostly?
      if i reach later n can not catch in a timely manner, would in not be better if i had done vaginal insemination just so i do not miss the egg?

      pls let me know.thanks a ton in advance.

        The hCG shot does not guarantee ovulation, but if the follicle is ripe and the blood estradiol level is high enough and the LH is still low and the progesterone level is still low, the hCG shot should almost assure ovulation will occur about 36 hrs. later. While one can consider vaginal insemination to assure not missing the mark, why not just assure the IUI is timed properly to ovulation???

        I hope your RE outlined the average success rate of natural cycle donor sperm/IUI in your age group with you...

          Hi Dr Roseff,

          Thank you for yr eply.
          My re put me on siphene and aloe compund for cd3-cd7 and then had u/s on cd10 today n was told there is no dominant follicle. she told me to come after 3days again for u/s. Pls advise if the Dom follicle selection would hv taken place by then? I am a bit confused as I do ovulate on my own.
          Could it be that I already ovulated on CD8?? As I have short cycles of 25 to 28 days? Waiting for your inputs..

            Hi Kiran,

            If one ovulates on her own without medication, then there's generally no point/utility in taking clomiphene (or similar pills). Aloe does nothing for fertility. Giving clomiphene to someone who already ovulates without it can "throw a wrench into the works"!

              1-Is there anyway i could reverse the effect of siphene?
              2-willthe dominant follicle selection take place eventually or if it has not taken place by cd10 it would be an anovulatory cycle? is there a time limit for dominant follicle selection?
              3-I was told I also have endometrial polyps 6mmx10mm. should i get it operated?

                ==> Most times a follicle will become dominant on clomiphene by cycle day 17.

                ==> Can't tell you what to do with this (it's illegal since you're not my patient). Sorry...
